We are seeking a dedicated and passionate Qualified Social Worker to join our Children's SW - Transition Team in Dudley. This role is pivotal in supporting children and young people as they transition between different stages of their lives, ensuring they receive the appropriate guidance and care. As a member of our team, you will work collaboratively with families, schools, and other agencies to facilitate smooth transitions and promote positive outcomes for children.
Your key responsibilities will include:
Conducting assessments to identify the needs of children and young people during transitional phases. Developing and implementing tailored support plans in collaboration with families and multidisciplinary teams. Providing direct support and guidance to children and their families, ensuring their voices are heard and respected. Monitoring and reviewing progress, making necessary adjustments to support plans as needed. Maintaining accurate and up-to-date case records and documentation in compliance with local policies and procedures.The ideal candidate will possess the following essential qualifications and skills:
A recognized degree in Social Work and registration with Social Work England. Experience working with children and families, particularly in transition-related contexts. Strong assessment and analytical skills, with the ability to develop effective support strategies. Excellent communication and interpersonal skills, enabling you to build rapport with children and families.
