We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Hospital Team in Barnet. This role is vital in ensuring that adults receive the support and care they need during their hospital stay and upon discharge. You will work collaboratively with healthcare professionals, patients, and their families to assess needs, develop care plans, and facilitate access to services.
