Advanced Prac - Leaving Care Team - Harrow

We are seeking a dedicated and experienced Advanced Practitioner to join our Leaving Care Team in Harrow. This pivotal role involves providing high-quality support and guidance to care leavers, ensuring they transition successfully into independent living. You will work collaboratively with young people, their families, and various stakeholders to facilitate access to education, employment, and housing services.

Your key responsibilities will include:

  • Assessing the needs of young people leaving care and developing tailored support plans.
  • Providing direct support and advice to young people, helping them navigate challenges and achieve their goals.
  • Collaborating with social workers, local authorities, and community organizations to coordinate services.
  • Monitoring and reviewing the progress of individuals in the leaving care program.
  • Training and mentoring less experienced staff, sharing best practices in supporting care leavers.

To be considered for this role, you must possess:

  • A relevant social work qualification (BA/MA in Social Work or equivalent).
  • Registration with Social Work England.
  • A minimum of 3 years’ experience working with young people in a social work setting, preferably within leaving care.
  • Strong knowledge of legislation and best practices related to leaving care.
  • Excellent communication and interpersonal skills, with the ability to build rapport with young people from diverse backgrounds.
