We are seeking a dedicated and compassionate Social Worker to join our Adults Communities team in Sheffield. In this role, you will be responsible for providing high-quality social work services to adults in need, ensuring their well-being and promoting independence. You will work collaboratively with individuals, families, and other professionals to assess needs, develop care plans, and implement interventions that support positive outcomes.
Your key responsibilities will include:
Conducting comprehensive assessments of clients’ needs, strengths, and risks. Developing and reviewing care plans in partnership with clients and their families. Providing guidance and support to clients, helping them access necessary services and resources. Collaborating with multidisciplinary teams to ensure cohesive service delivery. Maintaining accurate and timely documentation in accordance with legal and organizational standards. Advocating for clients' rights and needs within the community.To be considered for this position, you must hold a relevant Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent) and be registered with Social Work England. Additionally, you should possess strong communication and interpersonal skills, along with the ability to work effectively under pressure. Experience in adult social work, particularly in community settings, is highly desirable.
