In this role, you will work collaboratively with healthcare professionals to deliver high-quality social care services to adults, ensuring they receive the support they need during their hospital stay and subsequent discharge.
Your key responsibilities will include:
Conducting comprehensive assessments of patients' social care needs. Developing and implementing care plans in collaboration with multidisciplinary teams. Providing emotional support and guidance to patients and their families. Facilitating access to community resources and services upon discharge. Maintaining accurate and up-to-date case records and documentation.To be considered for this position, you must possess the following essential qualifications and skills:
A degree in Social Work and registration with Social Work England. Proven experience in adult social work, preferably in a hospital environment. Strong assessment and analytical skills, with the ability to make informed decisions. Excellent communication and interpersonal skills to effectively engage with patients and colleagues.
