Your key responsibilities will include:

  • Conducting comprehensive assessments of individuals in both community and hospital settings.
  • Developing and implementing care plans in collaboration with service users and other professionals.
  • Ensuring safeguarding procedures are adhered to and promoting the welfare of individuals.
  • Providing ongoing support and advocacy for service users to facilitate their independence.
  • Maintaining accurate and up-to-date case records in compliance with local authority policies.

To be considered for this role, you must possess the following essential qualifications and skills:

  • A recognized social work qualification (Degree in Social Work, DipSW, or equivalent).
  • Current registration with Social Work England.
  • At least 2 years of post-qualification experience in a similar setting.
  • Strong assessment, communication, and interpersonal skills.
  • A thorough understanding of relevant legislation and safeguarding practices.
