In this role, you will provide essential support and intervention to individuals and their families, ensuring they receive the appropriate care and resources to improve their well-being. Your expertise will contribute to the development of personalized care plans and collaborative approaches to mental health treatment.
Your key responsibilities will include:
Conducting comprehensive assessments of clients' needs and mental health conditions. Developing and implementing care plans in collaboration with clients, families, and multidisciplinary teams. Providing ongoing support and guidance to clients, helping them navigate mental health services and community resources. Monitoring and evaluating client progress, adjusting care plans as necessary. Maintaining accurate and up-to-date records in compliance with local and national standards.To be successful in this role, you must possess the following qualifications and skills:
A recognized social work degree and current Social Work England registration. Proven experience in adult social work, particularly within mental health settings. Strong knowledge of mental health legislation, policies, and best practices. Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
