In this role, you will assess needs, develop care plans, and collaborate with multidisciplinary teams to provide comprehensive support tailored to each individual.
Your key responsibilities will include:
Conducting thorough assessments of clients’ mental health and social needs. Developing and implementing personalized care plans in collaboration with clients and their families. Providing ongoing support and intervention, ensuring clients have access to necessary resources. Working closely with healthcare professionals, community organizations, and other stakeholders to coordinate care. Maintaining accurate and up-to-date records in compliance with local and national regulations.To be successful in this role, you will need to possess the following essential qualifications and skills:
A recognized social work qualification (e.g., DipSW, BA Hons in Social Work, or equivalent). Current registration with Social Work England. Experience working with adults with mental health issues, demonstrating a strong understanding of mental health legislation and best practices. Excellent communication and interpersonal skills, with the ability to build rapport with clients and their families.
