Join our dedicated team as a Trafford Adults Social Worker in the Community Team, where you will play a vital role in supporting vulnerable adults to lead fulfilling lives. In this position, you will work collaboratively with individuals, families, and partner agencies to assess needs and deliver tailored support services. Your key responsibilities will include:
Conducting comprehensive assessments of individuals’ needs, strengths, and risks. Developing and implementing care plans in partnership with service users and their families. Providing ongoing support, monitoring, and reviewing care plans to ensure effectiveness. Collaborating with health and social care professionals to coordinate services. Advocating for the rights and interests of adults in need of support.To be successful in this role, you must possess the following essential qualifications and skills:
A relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in adult social work, preferably within a community setting. Strong assessment and analytical skills, with the ability to develop person-centered plans.
