We are seeking a dedicated and compassionate professional who is committed to delivering high-quality social work services. In this role, you will work collaboratively with individuals, families, and other professionals to assess needs, develop care plans, and ensure the well-being of those we serve.
Your key responsibilities will include:
Conducting comprehensive assessments of individuals' social and emotional needs. Developing, implementing, and reviewing care plans in partnership with service users and their families. Providing support and guidance to individuals and their families, ensuring they understand available services. Collaborating with multi-disciplinary teams and external agencies to coordinate care and support. Maintaining accurate and timely records in compliance with local policies and procedures. Participating in safeguarding processes to protect vulnerable adults from harm.To be considered for this position, you must possess the following qualifications and skills:
A relevant Social Work qualification (BA/MA in Social Work, DipSW, or equivalent). Registration with Social Work England. Experience working with adults in a social work capacity, ideally within a community setting. Strong assessment and analytical skills, with the ability to manage complex cases. Excellent communication and interpersonal skills, with a commitment to person-centered practice.
