Hackney Council is seeking a dedicated and compassionate Qualified Social Worker to join our Preparing for Adulthood Team. This pivotal role involves supporting young adults with disabilities as they transition into independent living and adulthood. You will work collaboratively with individuals, families, and multi-disciplinary teams to ensure the best possible outcomes for our service users.

Key responsibilities include:

  • Assessing the needs of young adults and developing tailored support plans.
  • Providing guidance and support to families and caregivers in navigating services and resources.
  • Facilitating person-centered planning meetings and ensuring the voice of the young adult is heard.
  • Coordinating with external agencies, educational institutions, and health services to promote integrated support.
  • Monitoring progress and adapting support plans as necessary to meet changing needs.

The ideal candidate will possess the following qualifications and skills:

  • A recognized social work qualification (BA/MA in Social Work or equivalent).
  • Current Social Work England registration.
  • Experience working with young adults with disabilities and understanding of the transition process.
  • Strong assessment and analytical skills, with the ability to develop effective support plans.
  • Excellent communication and interpersonal skills, demonstrating empathy and resilience.
