We are seeking a dedicated and compassionate Qualified Social Worker to join our Community Team in Sheffield, focusing on adult services. As a vital member of our team, you will play a crucial role in supporting adults in need, promoting their wellbeing, and enhancing their quality of life. Your expertise will contribute to the development and implementation of effective care plans, ensuring that individuals receive the appropriate support tailored to their unique circumstances.
Your key responsibilities will include:
Assessing the needs of adults and their families to provide comprehensive support. Developing and reviewing care plans in collaboration with service users and other professionals. Providing advocacy and support to individuals, ensuring their voices are heard. Working closely with multidisciplinary teams to coordinate services and resources. Maintaining accurate records and documentation in accordance with legal and organizational standards.To be successful in this role, you will need to possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in adult social work, particularly in community settings. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
