In this role, you will play a crucial part in assessing and reviewing the needs of adults, ensuring they receive the support and services necessary for their well-being and independence.

Your key responsibilities will include:

  • Conducting comprehensive assessments and reviews of adult service users to identify their needs and preferences.
  • Developing and implementing person-centered care plans in collaboration with service users and their families.
  • Working closely with multidisciplinary teams and external agencies to coordinate services and resources.
  • Advocating for the rights and best interests of service users, ensuring their voices are heard.
  • Maintaining accurate and up-to-date case records in compliance with legislation and organizational policies.

To be considered for this position, you must possess the following essential qualifications and skills:

  • A recognized Social Work qualification (e.g., BA/MA in Social Work).
  • Current registration with Social Work England.
  • Experience working with adults in a social work capacity, preferably in a review or assessment role.
  • Strong communication and interpersonal skills, with the ability to build rapport with service users and stakeholders.
  • Excellent organizational skills and the ability to manage a varied caseload effectively.
