We are currently seeking a dedicated and experienced Qualified Social Worker to join our Assessment Team in Denbighshire. In this role, you will be responsible for conducting assessments and providing support to individuals and families in need. Your key duties will include:

  • Undertaking comprehensive assessments to identify the needs and risks of service users
  • Developing and implementing care plans in collaboration with service users and their support networks
  • Providing ongoing support and intervention to promote the well-being and safety of vulnerable individuals
  • Working closely with other professionals and agencies to coordinate services and resources for service users

The ideal candidate for this position will possess the following qualifications and skills:

  • A recognized social work qualification and registration with Social Care Wales
  • Demonstrated experience in conducting assessments and working with vulnerable populations
  • Strong communication and interpersonal skills to engage effectively with service users and stakeholders
  • Ability to work autonomously and as part of a multidisciplinary team

In addition to the essential qualifications and skills, candidates with experience in child protection, mental health, or disability services will be highly regarded.
