Join our dedicated Locality Team in Newcastle as an Adult's Social Worker, where you will play a vital role in improving the lives of adults in need of social care support. In this challenging yet rewarding position, you will be responsible for assessing the needs of individuals, developing care plans, and providing ongoing support to ensure the best possible outcomes for service users. Your expertise will be crucial in collaborating with families, healthcare professionals, and community organizations to deliver holistic care and support.
Your key responsibilities will include:
- Conducting comprehensive assessments to identify the needs and preferences of adults requiring social care.
- Developing, implementing, and reviewing personalized care plans in collaboration with service users and their families.
- Providing information, advice, and support to individuals and their families regarding available services and resources.
- Working within legal frameworks and safeguarding policies to protect vulnerable adults.
- Maintaining accurate and up-to-date records of assessments, care plans, and interventions.
- Participating in multi-disciplinary team meetings and contributing to the overall development of the Locality Team.
To be considered for this role, you will need the following essential qualifications and skills:
- A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Proven experience in adult social work, particularly within a locality or community setting.
- Strong assessment and analytical skills with the ability to develop effective care plans.
- Excellent communication and interpersonal skills, with a compassionate approach to service users.
- Knowledge of relevant legislation and best practices in adult social care.
