Your key responsibilities will include:

  • Conducting comprehensive assessments to identify the needs and preferences of adults requiring social care.
  • Developing, implementing, and reviewing personalized care plans in collaboration with service users and their families.
  • Providing information, advice, and support to individuals and their families regarding available services and resources.
  • Working within legal frameworks and safeguarding policies to protect vulnerable adults.
  • Maintaining accurate and up-to-date records of assessments, care plans, and interventions.
  • Participating in multi-disciplinary team meetings and contributing to the overall development of the Locality Team.

To be considered for this role, you will need the following essential qualifications and skills:

  • A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Registration with Social Work England.
  • Proven experience in adult social work, particularly within a locality or community setting.
  • Strong assessment and analytical skills with the ability to develop effective care plans.
  • Excellent communication and interpersonal skills, with a compassionate approach to service users.
  • Knowledge of relevant legislation and best practices in adult social care.
