Key responsibilities include:

  • Conducting comprehensive assessments of individuals’ needs and circumstances.
  • Developing, implementing, and reviewing care plans in collaboration with service users and their families.
  • Providing ongoing support and guidance to individuals, ensuring they have access to necessary resources and services.
  • Working collaboratively with multidisciplinary teams, including health professionals, to achieve the best outcomes for service users.
  • Maintaining accurate and up-to-date case records and documentation in accordance with legal and organizational standards.

The ideal candidate will possess the following essential qualifications and skills:

  • A relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Current registration with Social Work England.
  • Experience in adult social work, particularly within a locality team setting.
  • Strong communication and interpersonal skills, with the ability to build rapport with service users.
  • Excellent problem-solving abilities and a commitment to safeguarding and promoting the welfare of vulnerable adults.
