We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work - Community Team in Oxfordshire. This is an exciting opportunity to make a meaningful impact in the lives of vulnerable adults within our community. As a key member of our team, you will be responsible for providing high-quality social work services, promoting independence, and enhancing the well-being of individuals and families.
Your key responsibilities will include:
Conducting comprehensive assessments of service users’ needs and risks. Developing and implementing care plans in collaboration with service users and their families. Providing ongoing support and guidance to individuals, ensuring they have access to necessary resources and services. Working closely with other professionals, agencies, and community organizations to facilitate integrated care. Maintaining accurate and up-to-date case records and documentation in accordance with local policies and procedures.To be considered for this role, you must possess the following essential qualifications and skills:
A recognized social work qualification (Degree in Social Work or equivalent). Registration with Social Work England. Experience working with adults in a social care setting. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
