Key responsibilities include:
- Conducting comprehensive assessments of individuals with learning disabilities to identify their needs and preferences.
- Collaborating with multi-disciplinary teams to create and implement tailored care plans.
- Providing guidance and support to clients and their families, ensuring they are informed and empowered in their care.
- Monitoring and reviewing care plans regularly to ensure they remain relevant and effective.
- Advocating for clients' rights and access to necessary services and resources.
To be successful in this role, candidates must possess the following qualifications and skills:
- A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Experience working with adults with learning disabilities, including knowledge of relevant legislation and best practices.
- Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
- Excellent organizational skills and the ability to manage a caseload effectively.
