Join our dedicated Community Team in Newham as an Adult's Social Worker, where your expertise will make a meaningful difference in the lives of vulnerable adults. In this role, you will be responsible for assessing the needs of individuals, developing tailored care plans, and coordinating services to promote independence and well-being. You will work collaboratively with various stakeholders, including healthcare providers, families, and community organizations, to ensure the highest quality of care is delivered.
Your key responsibilities will include:
- Conducting comprehensive assessments of adults’ social care needs.
- Developing and implementing person-centered care plans.
- Providing ongoing support and reviewing care plans regularly.
- Advocating for clients and facilitating access to necessary services.
- Maintaining accurate records and documentation in compliance with regulatory standards.
To be considered for this position, you must possess the following qualifications and skills:
- A relevant degree in Social Work and registration with Social Work England.
- Demonstrable experience working with adults in a social care setting.
- Strong knowledge of relevant legislation and best practices in social work.
- Excellent communication, interpersonal, and organizational skills.
- The ability to work effectively both independently and as part of a team.
