Your key responsibilities will include:
Conducting comprehensive assessments of adults experiencing mental health issues. Developing and implementing personalized care plans in collaboration with clients and multidisciplinary teams. Providing direct support and guidance to individuals, families, and caregivers. Monitoring progress and adjusting care plans as necessary to ensure optimal outcomes. Advocating for clients’ needs and facilitating access to community resources and services.The ideal candidate will possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Proven experience in adult social work, particularly in mental health settings. Strong interpersonal and communication skills, with the ability to build rapport with clients.
