Your key responsibilities will include:

  • Conducting comprehensive assessments of adults’ needs and risks, ensuring a person-centered approach.
  • Developing, implementing, and reviewing care plans in collaboration with service users and their families.
  • Providing direct support and interventions, advocating for individuals' rights and needs.
  • Working closely with healthcare professionals, community organizations, and other stakeholders to coordinate services.
  • Maintaining accurate and timely records in accordance with statutory requirements and organizational policies.

To be successful in this role, you will need the following qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Current Social Work England registration.
  • Experience in adult social work, particularly with vulnerable populations.
  • Strong assessment and analytical skills, with the ability to make informed decisions.
  • Excellent communication and interpersonal skills, with a compassionate approach to service users.
