We are seeking a dedicated and compassionate Adult's Social Services Worker (SSW) to join our Mental Health Team in Richmond & Wandsworth. This is an exciting opportunity for a qualified social worker to make a significant impact on the lives of adults facing mental health challenges. As a key member of our team, you will provide support, guidance, and advocacy to individuals, ensuring they receive the necessary care and resources to thrive.

Key responsibilities will include:

Assessing the needs of clients and developing tailored care plans. Providing direct support and intervention to individuals experiencing mental health issues. Collaborating with multidisciplinary teams, including healthcare professionals, to ensure holistic care. Facilitating access to community resources and services. Maintaining accurate case records and documentation in compliance with legal and organizational standards.

To be successful in this role, candidates must possess the following qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England. Experience working within mental health services or a related field. Strong assessment and intervention skills. Excellent communication and interpersonal abilities.

Join our dedicated Adulthood Team in Richmond & Wandsworth as a Qualified Social Worker, where you will play a vital role in improving the lives of adults in our community. In this rewarding position, you will work collaboratively with individuals, families, and multidisciplinary teams to assess needs, develop care plans, and provide ongoing support to enhance well-being and independence.

Your key responsibilities will include:

Conducting comprehensive assessments of adults’ needs and risks, ensuring a person-centered approach. Developing, implementing, and reviewing care plans in collaboration with service users and their families. Providing direct support and interventions, advocating for individuals' rights and needs. Working closely with healthcare professionals, community organizations, and other stakeholders to coordinate services. Maintaining accurate and timely records in accordance with statutory requirements and organizational policies.

To be successful in this role, you will need the following qualifications and skills:
