We are currently seeking a dedicated and experienced Adult's Social Worker to join our Community Team in Cardiff. As a key member of our team, you will be responsible for providing high-quality social work services to vulnerable adults in the community, ensuring their safety and well-being.

Key responsibilities and duties include:

  • Conducting assessments and reviews to identify the needs of service users
  • Developing care plans and support packages tailored to individual needs
  • Working collaboratively with other professionals and agencies to provide holistic support
  • Advocating for the rights and best interests of service users
  • Participating in multidisciplinary meetings and case conferences

The ideal candidate will possess the following essential qualifications and skills:

  • Qualified Social Worker with a valid registration with Social Care Wales
  • Demonstrated experience in adult social work, preferably within a community setting
  • Strong assessment and decision-making skills
  • Excellent communication and interpersonal skills
  • Ability to work effectively under pressure and manage a complex caseload
