As a member of our team, your key responsibilities will include:

  • Conducting comprehensive assessments of service users' needs and risks.
  • Developing and implementing person-centered care plans in collaboration with service users and their families.
  • Providing ongoing support and guidance to individuals, ensuring they have access to appropriate resources and services.
  • Working closely with multidisciplinary teams, including mental health professionals, to ensure holistic care.
  • Maintaining accurate and up-to-date case records in accordance with statutory requirements.

To be successful in this role, you will need to meet the following essential qualifications and skills:

  • A recognized social work qualification (e.g., BA/MA in Social Work).
  • Current Social Work England registration.
  • Experience working within mental health services, particularly with adults.
  • Strong assessment and analytical skills, with the ability to develop effective care plans.
  • Excellent communication and interpersonal skills, capable of building rapport with service users and stakeholders.
