We are currently seeking a dedicated and experienced Adult's Social Worker to join our Learning Disabilities Team in Lewisham. As a key member of the team, you will be responsible for providing high-quality support and intervention to adults with learning disabilities, enabling them to live fulfilling and independent lives.

Key responsibilities and duties include:

  • Assessing the needs of adults with learning disabilities and developing care plans tailored to their individual requirements.
  • Working collaboratively with multi-disciplinary teams, service users, and their families to deliver person-centered care and support.
  • Undertaking safeguarding investigations and implementing appropriate measures to protect vulnerable adults.
  • Advocating for the rights and best interests of service users, ensuring their voices are heard and respected.
  • Participating in case conferences, reviews, and team meetings to contribute to the continuous improvement of service delivery.

The ideal candidate will possess the following essential qualifications and skills:

  • Qualified Social Worker with a relevant social work qualification and HCPC registration.
  • Demonstrated experience in working with adults with learning disabilities within a social care setting.
  • Strong assessment and care planning skills, with the ability to engage effectively with service users and their support networks.
  • Excellent communication and interpersonal skills, with a compassionate and empathetic approach to supporting vulnerable individuals.
  • Sound knowledge of relevant legislation, policies, and procedures related to adult social care and safeguarding.

In addition to the essential qualifications and skills, candidates with experience in mental capacity assessments, MCA/DoLS applications, and working with complex cases will be highly desirable.
