Children's AP - Adolescent Team - Lewisham

We are seeking a dedicated and experienced Children's Advanced Practitioner to join our Adolescent Team in Lewisham. As a key member of our team, you will be responsible for providing high-quality social work services to children and families, with a focus on adolescents in challenging circumstances.

Key responsibilities and duties include:

  • Undertaking assessments and developing care plans for adolescents in need of support
  • Providing direct intervention and support to young people, including counseling and advocacy
  • Working collaboratively with multi-disciplinary teams to ensure the holistic well-being of adolescents
  • Participating in case conferences, court proceedings, and reviews to advocate for the best interests of the young people
  • Contributing to the development and implementation of policies and procedures to improve service delivery

  • Qualified Social Worker with a recognized social work qualification
  • Registered with Social Work England
  • Demonstrated experience in working with adolescents in a social work setting
  • Strong assessment, analytical, and decision-making skills
  • Excellent communication and interpersonal skills
