We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our team in Slough in a Hospital Discharge role. As a key member of our social work team, you will play a crucial role in facilitating safe and timely discharges for patients, ensuring they receive the necessary support and services upon leaving the hospital.

Key responsibilities and duties:

  • Assessing the needs of patients being discharged from the hospital and developing comprehensive discharge plans.
  • Liaising with healthcare professionals, community services, and external agencies to coordinate post-discharge support.
  • Providing advocacy and support to patients and their families to ensure a smooth transition from hospital to home or alternative care settings.
  • Completing all necessary documentation and reports in a timely and accurate manner.
  • Participating in multi-disciplinary meetings and case conferences to ensure a holistic approach to patient care.

Essential qualifications and skills:

  • Qualified Social Worker with a valid registration with Social Work England.
  • Demonstrated experience in a similar role, preferably within a hospital or healthcare setting.
  • Strong assessment and case management skills with the ability to work autonomously and as part of a team.
  • Excellent communication and interpersonal skills, with the ability to build rapport with patients and collaborate effectively with colleagues and external partners.
  • Knowledge of relevant legislation and policies related to social work practice in healthcare settings.
