Adult's SW - Learning Disability Team - Tower Hamlets
Join our dedicated Adult's Social Work - Learning Disability Team in Tower Hamlets, where you will play a vital role in enhancing the lives of adults with learning disabilities. As a Qualified Social Worker, you will be responsible for providing holistic assessments, developing tailored care plans, and ensuring the delivery of high-quality support services. Your key responsibilities will include:
- Conducting comprehensive assessments of individuals with learning disabilities to identify their needs and strengths.
- Developing, implementing, and reviewing person-centered care plans in collaboration with service users and their families.
- Working closely with multi-disciplinary teams, external agencies, and community resources to facilitate effective support.
- Providing guidance and support to families and carers, ensuring they are well-informed and involved in the care process.
- Maintaining accurate and up-to-date case records in compliance with local and national policies.
The ideal candidate will possess the following essential qualifications and skills:
- A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Current registration with Social Work England.
- Proven experience working with adults with learning disabilities, preferably within a social work setting.
- Strong communication and interpersonal skills, with the ability to build rapport with service users and stakeholders.
- Excellent organizational skills and the ability to manage a diverse caseload effectively.
