We are seeking a dedicated and compassionate Social Worker to join our Case Management team in Hackney, focusing on backlog cases. In this challenging yet rewarding role, you will be responsible for providing high-quality social work services to individuals and families, ensuring they receive the support and resources they need to thrive. Your expertise will be crucial in managing a diverse caseload and addressing the unique needs of our clients.

Your key responsibilities will include:

Conducting comprehensive assessments of clients' needs and circumstances. Developing and implementing tailored care plans in collaboration with clients and relevant stakeholders. Monitoring and reviewing clients' progress, adjusting care plans as necessary. Providing emotional support and practical advice to clients and their families. Coordinating with other professionals and agencies to ensure holistic care. Maintaining accurate and up-to-date case records in compliance with legislation and best practices.

The ideal candidate will possess the following qualifications and skills:

A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Proven experience in case management within a social work setting. Strong assessment and analytical skills. Excellent communication and interpersonal abilities. Ability to work independently and as part of a multidisciplinary team.
