We are seeking a dedicated and compassionate Adult Social Worker to join our dynamic team at a leading hospital in Norfolk for a 6-month contract. This role is crucial in providing social work support to adults within a hospital setting, ensuring they receive the care and resources they need during their recovery. The ideal candidate will have a strong commitment to improving patient outcomes and a passion for helping individuals navigate complex social and health-related challenges.

Key responsibilities include:

Conducting assessments of patients' social and emotional needs upon admission. Developing and implementing care plans in collaboration with multidisciplinary teams. Providing counseling and support to patients and their families, addressing emotional and practical concerns. Facilitating discharge planning, ensuring a smooth transition to home or alternative care settings. Advocating for patients’ rights and accessing community resources and services.

To be considered for this position, candidates must possess the following essential qualifications and skills:

A recognized degree in Social Work and current registration with Social Work England. Experience working in a hospital or healthcare environment, preferably with adult clients. Strong assessment and intervention skills, with the ability to work under pressure. Excellent communication and interpersonal skills, with a compassionate approach to patient care. Knowledge of relevant legislation and policies affecting adult social care.
