Are you a dedicated and compassionate social worker looking to make a significant impact in the lives of adults within the Hackney community? We are seeking a qualified Adult's Social Worker to join our Case Management Team. In this pivotal role, you will be responsible for assessing the needs of individuals, developing tailored support plans, and coordinating services to ensure the best possible outcomes for our clients.

Your key responsibilities will include:

Conducting comprehensive assessments of clients' needs, strengths, and risks. Developing and implementing care plans in collaboration with clients and their families. Coordinating with multidisciplinary teams, including healthcare providers and community organizations. Monitoring and reviewing client progress to adjust support plans as necessary. Providing advocacy and support to clients in navigating social services and resources.

To be successful in this role, you must possess the following essential qualifications and skills:

A recognized social work qualification (e.g., BA/MA in Social Work, CQSW, DipSW). Current registration with Social Work England. Strong knowledge of relevant legislation and best practices in adult social work. Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders. Proficiency in case management software and documentation practices.
