  • Collaborating with multidisciplinary teams to develop and implement person-centered care plans.

  • Providing advice and support to families and carers regarding available services and resources.
  • Maintaining accurate records and documentation in compliance with local and national policies.

To be considered for this position, you must possess the following essential qualifications and skills:

  • A recognized social work qualification (BA/MA in Social Work or equivalent).
  • Current registration with Social Work England.
  • Proven experience in adult social care, particularly with Care Act assessments.
  • Strong communication and interpersonal skills, with the ability to build rapport with service users.
  • Excellent organizational skills and the ability to manage a varied caseload effectively.
