Assessment Social Worker - Middlesbrough £36.61
We are seeking a dedicated and experienced Assessment Social Worker to join our team in Middlesbrough. As a key member of our Qualified Social Work field, you will be responsible for conducting thorough assessments of individuals and families to determine their needs and develop appropriate support plans.
Key responsibilities and duties:
- Conduct comprehensive assessments to identify risks and protective factors
- Develop and implement care plans in collaboration with service users and other professionals
- Provide support and guidance to individuals and families to improve their well-being and quality of life
- Participate in multi-disciplinary meetings and case conferences to ensure coordinated service delivery
- Maintain accurate and up-to-date case records and reports
Essential qualifications and skills:
- Qualified Social Worker with a relevant degree and HCPC registration
- Demonstrated experience in conducting assessments and developing care plans
- Strong communication and interpersonal skills
