We are currently seeking a dedicated and experienced Qualified Social Worker to join our Children with Disabilities (CWD) Team in Wiltshire. As a vital member of our team, you will be responsible for providing high-quality support and intervention services to children and families in need.

Key responsibilities and duties:

  • Assessing the needs of children with disabilities and their families to develop and implement care plans
  • Providing direct support and guidance to children and families to promote their well-being and safeguarding
  • Collaborating with multi-disciplinary teams, external agencies, and partners to ensure coordinated and effective service delivery
  • Participating in case conferences, reviews, and court proceedings as required
  • Maintaining accurate and up-to-date case records and reports

Essential qualifications and skills:

  • Qualified Social Worker with a valid HCPC registration
  • Experience working with children with disabilities and their families
  • Strong assessment, intervention, and case management skills
  • Excellent communication, interpersonal, and teamwork abilities
  • Ability to work effectively under pressure and manage competing priorities
