We are currently seeking a dedicated and passionate individual to join our team as a Children's TM - MACH Team - Middlesbrough in the Qualified Social Work field. As part of this role, you will be responsible for providing high-quality social work services to children and families in need within the Middlesbrough area.

Key responsibilities and duties include:

Conducting assessments and developing care plans for children and families Participating in multi-disciplinary team meetings to collaborate on case management Providing direct support and intervention to children and families to promote positive outcomes Working closely with external agencies and partners to ensure holistic support for clients Keeping accurate and up-to-date case records in compliance with regulatory standards

The ideal candidate for this position will possess the following essential qualifications and skills:

Qualified Social Worker with relevant experience in children's services Registered with the Health and Care Professions Council (HCPC) Demonstrated ability to work effectively in a fast-paced and challenging environment Strong communication and interpersonal skills Ability to build rapport with children and families from diverse backgrounds
