We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our team at a leading hospital in Shropshire. As a vital member of the healthcare team, the successful candidate will play a key role in providing support and interventions to patients and their families to ensure their well-being and safety.
Key responsibilities and duties:
Conduct comprehensive assessments of patients' social care needs and develop care plans accordingly. Provide practical and emotional support to patients and their families, offering guidance on coping strategies and accessing community resources. Work collaboratively with other healthcare professionals, including doctors, nurses, and therapists, to ensure holistic care for patients. Advocate on behalf of patients to ensure their rights are upheld and that they receive the appropriate support and services. Maintain accurate and up-to-date records of interventions and progress, ensuring confidentiality and compliance with data protection regulations.Essential qualifications and skills:
Qualified Social Worker (QSW) registration with the Health and Care Professions Council (HCPC). Demonstrated experience in a hospital or healthcare setting, preferably within a social work capacity. Strong communication and interpersonal skills, with the ability to build rapport with patients and colleagues. Excellent assessment and decision-making abilities, with a focus on person-centered care.
