We are currently seeking a dedicated and experienced Adult's Social Worker to join our Hospital Team in North Lincolnshire. As a key member of our team, you will be responsible for providing high-quality social work services to adults in a hospital setting.

Key responsibilities and duties include:

Conducting assessments to determine the needs of adult service users Developing and implementing care plans in collaboration with multidisciplinary teams Providing support and guidance to individuals and their families Advocating for the rights and best interests of service users Participating in safeguarding procedures and risk assessments

The ideal candidate will possess the following essential qualifications and skills:

Qualified Social Worker with a relevant degree and HCPC registration Demonstrated experience in adult social work, preferably within a hospital or healthcare setting Strong assessment and care planning skills Excellent communication and interpersonal abilities Ability to work effectively within a multidisciplinary team
