Nottingham City - Social Worker - Hospital Discharge / (D2A) Team
Nottingham City is seeking a dedicated and compassionate Social Worker to join our Hospital Discharge / Discharge to Assess (D2A) Team. Key responsibilities include:
- Conducting comprehensive assessments of clients’ needs and circumstances.
- Collaborating with healthcare professionals, families, and community services to develop and implement discharge plans.
- Providing information and support to clients and their families, ensuring they understand the discharge process and available resources.
- Monitoring and reviewing care plans post-discharge to ensure ongoing support and adjustments are made as necessary.
- Maintaining accurate and timely records in compliance with legal and organizational standards.
To be successful in this role, you must possess the following qualifications and skills:
- A relevant Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Experience in hospital discharge processes or similar settings.
- Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
- Excellent problem-solving abilities and a proactive approach to care coordination.
