We are currently seeking a Qualified Social Worker (QSW) to join the Hospital Discharge Team in Haringey. As a key member of the Hospital Discharge Team, your responsibilities will include:
- Conducting comprehensive assessments of patients' needs and risks to facilitate safe discharges.
- Collaborating with medical staff, families, and community services to create discharge plans tailored to individual needs.
- Providing information, advice, and support to patients and their families regarding available services and resources.
- Monitoring and reviewing post-discharge outcomes to ensure the effectiveness of care plans and interventions.
- Maintaining accurate and up-to-date case records in compliance with local policies and legal requirements.
To be considered for this role, candidates must possess the following qualifications and skills:
- A relevant Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Proven experience in hospital discharge processes and working within a multidisciplinary team.
- Strong communication and interpersonal skills, with the ability to build rapport and trust with patients and families.
- Excellent organizational skills and the ability to manage a caseload effectively.
