Join our dedicated Locality Team in Hull as a Qualified Social Worker, where you will play a vital role in supporting adults within the community. In this position, you will be responsible for assessing the needs of individuals, developing tailored support plans, and ensuring that vulnerable adults receive the assistance they require to lead fulfilling lives. Your expertise will be essential in delivering high-quality social work services that promote independence and well-being.
Your key responsibilities will include:
- Conducting comprehensive assessments and reviews of adult service users to identify their needs and risks.
- Developing and implementing person-centered support plans in collaboration with service users and their families.
- Working closely with multi-disciplinary teams and external agencies to coordinate effective care and support.
- Maintaining accurate and up-to-date case records, ensuring compliance with local policies and statutory requirements.
- Providing guidance and support to unqualified staff and students, fostering a learning environment.
To be considered for this role, you must possess the following qualifications and skills:
- A relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Proven experience in adult social work, particularly within a locality team setting.
- Strong assessment and analytical skills, with the ability to make informed decisions.
- Excellent communication and interpersonal skills, enabling effective collaboration with service users and colleagues.
