Lincolnshire County Council is seeking a motivated Children's Social Worker for our Leaving Care Team. As a Children's Social Worker in the Leaving Care Team, your key responsibilities will include:
Assessing the needs of young people leaving care and developing tailored support plans. Working collaboratively with young people, their families, and other professionals to facilitate a smooth transition to independent living. Providing guidance on life skills, education, employment, and housing options. Monitoring progress and reviewing plans regularly to ensure the best outcomes for young people. Maintaining accurate case records and documentation in compliance with local policies and procedures.To be considered for this role, you must possess the following qualifications and skills:
A recognized social work qualification (BA/MA in Social Work or equivalent). Current Social Work England registration. Experience working with children and young people, particularly in a leaving care or similar setting. Strong communication and interpersonal skills, with the ability to build rapport with young people.
