Join our dynamic Changing Futures Team within Adults Social Care as a Hull Social Worker, where your expertise will play a pivotal role in transforming the lives of vulnerable adults in our community. In this fulfilling position, you will be responsible for assessing the needs of service users, developing tailored care plans, and implementing effective interventions to promote independence and wellbeing. Collaborating with multidisciplinary teams, you will advocate for the rights and needs of individuals, ensuring they receive the support required to thrive.

Key responsibilities include:

Conducting comprehensive assessments of service users’ needs and risks. Developing and reviewing care plans in partnership with service users and their families. Providing ongoing support and guidance to individuals, helping them navigate complex systems. Maintaining accurate records and documentation in compliance with regulatory standards. Participating in team meetings and contributing to the continuous improvement of services.

To be considered for this role, you must possess a relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent) and be registered with Social Work England. Essential skills include strong communication and interpersonal abilities, a compassionate approach to service delivery, and a solid understanding of safeguarding policies and procedures. Experience in adult social care and knowledge of relevant legislation will be highly advantageous.
