Your key responsibilities will include:
- Conducting comprehensive assessments of clients' needs and circumstances.
- Developing and implementing tailored care plans in collaboration with clients and other professionals.
- Providing emotional support and practical assistance to clients and their families.
- Advocating for clients' rights and ensuring access to appropriate services.
- Maintaining accurate and up-to-date records and reports in accordance with legislative requirements.
- Participating in multidisciplinary team meetings and contributing to service development initiatives.
To be considered for this role, you must possess a recognized social work qualification (e.g., Degree in Social Work or equivalent) and be registered with Social Care Wales. We are looking for candidates who demonstrate strong interpersonal skills, a compassionate approach, and the ability to work effectively in a fast-paced environment. Experience in adult services and knowledge of relevant legislation will be advantageous.
