Your key responsibilities will include:

Conducting comprehensive assessments and evaluations of clients' needs. Developing and implementing individualized care plans in collaboration with clients and their families. Providing ongoing support and monitoring progress to ensure effective service delivery. Collaborating with multidisciplinary teams, external agencies, and community resources. Maintaining accurate records and documentation in compliance with local policies and regulations.

To be successful in this role, you must possess the following essential qualifications and skills:

A recognized social work qualification (e.g., CQSW, DipSW, or equivalent). Current registration with Social Care Wales. Demonstrated experience in adult social work, preferably in community settings. Strong communication and interpersonal skills, with the ability to build rapport with clients. Excellent organizational skills and the capacity to manage a varied caseload.
