Join our dedicated team in Blackpool as a Social Worker within the Community Adult Team. We are seeking an enthusiastic and compassionate individual who is committed to enhancing the lives of adults in our community. In this role, you will be responsible for assessing the needs of vulnerable adults, developing personalized care plans, and facilitating access to essential services. Your expertise will play a crucial role in safeguarding individuals and promoting their independence and well-being.
Your key responsibilities will include:
Conducting comprehensive assessments of clients’ needs and circumstances. Developing and implementing tailored care plans in collaboration with clients and their families. Providing ongoing support and monitoring progress to ensure effective service delivery. Working closely with multidisciplinary teams and external agencies to coordinate care. Advocating for clients' rights and ensuring their voices are heard in decision-making processes.To be considered for this position, you must possess:
A recognized social work qualification (e.g., Degree in Social Work, CQSW, DipSW). Registration with Social Work England. Demonstrable experience in adult social work, particularly within community settings. Strong communication and interpersonal skills, with the ability to build rapport with clients.
