Key responsibilities and duties:
- Conducting assessments and developing care plans for service users
- Working collaboratively with other professionals and agencies to ensure holistic support
- Providing direct intervention and support to individuals in need
- Participating in case conferences and reviews to monitor progress and make necessary adjustments
- Advocating for the rights and well-being of service users
Essential qualifications and skills:
- Qualified Social Worker with current registration with the Health and Care Professions Council (HCPC)
- Demonstrated experience in working within a social work setting, preferably within a locality team
