Adult's SW - Locality Team - Liverpool

We are currently seeking a dedicated Adult's Social Worker to join our Locality Team in Liverpool. As a key member of our team, you will be responsible for providing high-quality social work services to adults in need of support and intervention.

Key responsibilities and duties include:

  • Conducting assessments to determine the needs and risks of adults in the community
  • Developing and implementing care plans to address the identified needs of service users
  • Working collaboratively with multi-disciplinary teams and partner agencies to provide holistic support
  • Participating in safeguarding investigations and taking appropriate action to protect vulnerable adults
  • Advocating for service users to ensure their voices are heard and their rights are upheld

The ideal candidate for this role will possess:

  • A recognized Social Work qualification and registration with Social Work England
  • Demonstrated experience in adult social work, preferably within a locality team setting
  • Strong assessment and analytical skills to effectively identify needs and risks
  • Excellent communication and interpersonal skills to engage with service users, families, and stakeholders
  • Ability to work autonomously and as part of a team in a fast-paced environment
