We are currently seeking a dedicated and experienced Adult's Social Worker to join our Learning Disabilities Team in Somerset. As a key member of the team, you will be responsible for providing high-quality care and support to adults with learning disabilities, ensuring their well-being and promoting their independence.

Key responsibilities and duties include:

  • Assessing the needs of individuals with learning disabilities and developing care plans tailored to their specific requirements.
  • Working collaboratively with other professionals, such as healthcare providers and support workers, to ensure a holistic approach to care.
  • Advocating for the rights and best interests of clients, ensuring they receive the support and services they are entitled to.
  • Regularly reviewing and monitoring the progress of individuals, adjusting care plans as necessary to meet changing needs.

The ideal candidate will possess the following essential qualifications and skills:

  • Qualified Social Worker with a relevant degree and registration with the HCPC.
  • Experience working with adults with learning disabilities in a social care setting.
  • Strong assessment and care planning skills, with the ability to work autonomously and make informed decisions.
  • Excellent communication and interpersonal skills, with the ability to build rapport with clients and their families.

Additional requirements for this role include a valid driver's license and access to a vehicle, as home visits may be required.
