Adult's SW - MH Team - Sheffield
An opportunity is available for a qualified Adult Social Worker to join a Mental Health Team in Sheffield. You will support adults experiencing a range of mental health needs, helping them to achieve greater independence, improve their wellbeing and access appropriate services within their community.
Working as part of a multidisciplinary team, you will complete holistic assessments, identify eligible care and support needs, and develop outcome-focused care and support plans. You will coordinate services, make referrals to relevant professionals and community resources, and review arrangements regularly to ensure they remain effective and proportionate.
The role will involve safeguarding adults, managing risk, undertaking mental capacity assessments and contributing to best-interest decisions where required. You will work collaboratively with service users, carers, health professionals and other agencies, ensuring that people are treated with dignity, respect and in accordance with relevant legislation, policies and professional standards.
You will maintain accurate and timely case records, prepare reports and contribute to service planning and continuous improvement. The post may also involve supporting people with complex or changing circumstances, responding to urgent concerns and working flexibly to meet the needs of the service.
Applicants must hold a recognised Social Work qualification and current registration with Social Work England. Experience of working with adults in a mental health, social care or community setting is highly desirable, alongside knowledge of the Care Act, Mental Capacity Act, safeguarding procedures and strengths-based practice.
You should be confident managing a varied caseload, making sound professional judgements and working independently while contributing positively to a wider team. Excellent communication, organisation and report-writing skills are essential, as is the ability to build effective relationships with people from diverse backgrounds. A commitment to anti-discriminatory practice, person-centred support and positive outcomes is expected. A valid driving licence and access to transport may be required, depending on service needs.
