We are seeking a qualified Adult Social Worker to join a Mental Health and Locality Team supporting adults to live safely, independently and with greater control over their lives. This is an opportunity to work in a collaborative, community-focused environment where strengths-based practice, recovery and positive outcomes are at the heart of service delivery.

You will manage a varied caseload involving adults with mental health needs, including individuals who may have complex circumstances, fluctuating capacity or significant risks. Your responsibilities will include completing proportionate assessments under the Care Act, identifying eligible needs, developing outcome-focused support plans and arranging appropriate services. You will also review care and support arrangements, monitor outcomes, respond to safeguarding concerns and ensure that people’s views, wishes and desired outcomes remain central to decision-making.

The role involves working closely with adults, carers, families, health professionals, housing providers, voluntary organisations and other partner agencies. You will contribute to multi-disciplinary assessments, risk management and discharge planning, while promoting independence, community connection and least restrictive options. Accurate, timely recording and sound professional judgement will be essential, alongside the ability to manage competing priorities and respond effectively to changing circumstances.

Applicants must hold a recognised Social Work qualification and be registered with Social Work England. You should have experience of working with adults in a social care, mental health or related statutory setting, with a sound understanding of the Care Act, safeguarding principles, mental capacity legislation and human rights. We are looking for someone who communicates confidently, builds respectful professional relationships and can work constructively with people from diverse backgrounds.
