We are currently seeking a dedicated and experienced Long Term Social Worker to join our team in Gateshead. As a key member of our Qualified Social Work field, you will be responsible for providing high-quality support and intervention to vulnerable individuals and families.
Key responsibilities and duties:
- Conducting assessments to identify the needs and risks of service users.
- Developing and implementing care plans to address the identified needs and risks.
- Working collaboratively with other professionals and agencies to ensure holistic support for service users.
- Regularly reviewing and updating care plans to monitor progress and make necessary adjustments.
- Advocating for the rights and best interests of service users in various settings.
Essential qualifications and skills:
- Qualified Social Worker with a relevant degree and HCPC registration.
- Demonstrated experience in working with vulnerable individuals and families in a social work setting.
- Strong assessment and analytical skills to identify needs and risks accurately.
- Excellent communication and interpersonal skills to engage effectively with service users and stakeholders.
- Ability to work autonomously and as part of a multidisciplinary team.
